As a patient of Azorna Healthcare, you have a right to be notified in writing of your rights and obligations before care is provided. Hospice must protect and promote the exercise of these rights. Your family or guardian may exercise your rights if you lack the capacity to do so.
On the initial evaluation visit a Hospice team member will present the patient and/or caregiver with a copy of these rights and responsibilities. The Hospice Nurse will inform the patient and/or caregiver that the administrative nurse on call can be reached 24 hours a day, 7 days a week by calling Azorna Healthcare at (480) 637-4100.
As a Azorna Healthcare patient, you have the right:
To be fully informed, prior to or at the time of admission, of your rights as a patient to choose this agency from among all care providers and to choose your attending physician;
To be fully informed in terms that you can understand, at the time of admission, of your diagnosis and prognosis, treatment, alternatives to care, and plan of care; and to be presented choices about Advance Directives;
To be fully informed of: The types of services and service frequencies available from Azorna Healthcare and the services, disciplines, and service frequencies that you are to receive, Any risks involved, Related charges, including any charges for services not covered by third-party payers and the Azorna Healthcare basic rate, and How to manage emergencies;
To fully participate in the development and revisions of your plan of care.
To adequate, appropriate and timely care and services, and to receive effective pain management and symptom control from Azorna Healthcare for conditions related to the terminal illness.
To be assured of the identity and professional status of Azorna Healthcare and agency staff and that the staff who provide care are qualified through education and experience and are duly licensed to carry out the services for which they are responsible.
Not to be discriminated against based on race, national origin, religion, gender, sexual orientation, age, disability, marital status, or diagnosis;
To receive treatment that supports and respects the patient’s individuality, choices, strengths, and abilities;
To receive privacy in treatment and care for personal needs;
To review, upon written request, the patient’s own medical record according to A.R.S. §§ 12-2293, 12- 2294, and 12-2294.01;
To receive a referral to another health care institution if the hospice inpatient facility is not authorized or not able to provide physical health services needed by the patient;
To participate or have the patient’s representative participate in the development of, or decisions concerning, treatment;
To participate or refuse to participate in research or experimental treatment; and
To receive assistance from a family member, the patient’s representative, or other individual in understanding, protecting, or exercising the patient’s rights.
Azorna Healthcare has the responsibility to ensure:
Each patient is treated with dignity, respect, and consideration;
Each patient is not subjected to: abuse, neglect, exploitation, coercion, manipulation, sexual abuse, sexual assault, seclusion, restraint, retaliation for submitting a complaint to the Department or another entity; or misappropriation of personal and private property by the hospice’s personnel members, employees, volunteers, or students;
A patient or the patient’s representative: except in an emergency, either consents to or refuses treatment; may refuse or withdraw consent for treatment before treatment is initiated; is informed of proposed treatment alternatives, associated risks, and possible complications;
Consents to photographs of the patient before the patient is photographed, except that a patient may be photographed when admitted to a hospice for identification and administrative purposes;
Except as otherwise permitted by law, written consent to the release of information in the patient’s medical record or financial records is provided;
Each patient Is informed of: the components of hospice services provided by the hospice; the rates and charges for the components of hospice services before the components are initiated and before a change in rates, charges, or services; The hospice’s policy on health care directives; and the patient complaint process; and is informed that a written copy of rates and charges, as required in A.R.S. § 36- 436.03, may be requested.
